This checklist outlines the steps required to submit and process requests for maintenance in the office, ensuring a timely response to maintenance issues.

• Identify Maintenance Issue

  Assess and clearly identify the maintenance issue that needs attention, such as plumbing, electrical, or structural problems.

• Fill Out Maintenance Request Form

  Complete the maintenance request form, providing detailed information about the issue, including location, description, and urgency.

• Submit Request

  Submit the filled maintenance request form to the designated facilities management or administrative contact.

• Acknowledge Receipt

  Ensure that the receipt of the maintenance request is acknowledged by the facilities management team to confirm they have received the request.

• Schedule Maintenance

  Facilities management will schedule the maintenance work based on urgency and availability of maintenance personnel.

• Complete Maintenance Work

  The maintenance team will carry out the necessary work as scheduled and will notify the requestor upon completion.

• Follow-Up

  Conduct a follow-up to ensure that the maintenance work was completed satisfactorily and that the issue has been resolved.

• Close Request

  Document the completion of the maintenance request in the maintenance tracking system and notify the requestor that the issue has been resolved.
